Name changing: if you’re trying to change the default name, update the value next to the Default label. This does not impact that UUID in the database, which I think is what you’re concerned with?
Adding a new name allows you to provide another label (such as Nickname) and an associated value.
Dates: Those are not default values. Those are examples of the format that needs to be presented. There is no date associated with an individual unless a user enters one.
