What is the entire URL out of the browser, exactly where the error occurred?

Can you describe what the issue is you’re experiencing? I just accidentally deleted an encounter…577dfe41-b5b5-4427-bdb0-edda76fd4bd3
really need this restored please date was 10 Oct 2022, from location Bridgport by Matt Testoni.

Can you provide steps on how to reproduce what you’re experiencing?

I was able to recreate the Encounter here (at least photo and date):

I could not find a study site for Bridgport. Does that need to be addeD?
