Unable to create new organization

I’m trying to set up a new org on Wild North - FWSCalif. Despite getting a “Done successfully” message, the org still does not exist in WN:

I wasn’t able to reproduce the error, but I do see that the org is in the list now after trying to add it myself:

[ACWadmin1]

Weird. Must be something I did wrong. Thanks!
